Financial Eligibility Requirements

To qualify for IVF financial assistance through this program, applicants must have a combined annual household income of less than $50,000.

Applicants may be required to provide documentation verifying household income, including:

• Recent federal tax returns
• W-2 or 1099 forms
• Recent pay stubs
• Documentation of unemployment, disability or other household income
• Information regarding the income of a spouse or domestic partner
  
  

Eligibility is based on the combined gross income of all adults contributing financially to the household, not solely the income of the individual receiving treatment.

Meeting the income requirement does not guarantee selection. Applicants must also meet the medical, residency and treatment eligibility requirements established by Assisted Fertility Program and Scarlett’s Babies.

Who Should Apply?

You may qualify when:

• Your combined annual household income is less than $50,000
• IVF has been recommended as an appropriate treatment option
• The cost of fertility treatment has prevented or delayed you from moving forward
• You are willing to provide documentation of your financial circumstances
• You can travel to Assisted Fertility Program for required appointments and treatment
• You meet the medical requirements for IVF treatment

All financial information will be reviewed confidentially. Applicants who provide incomplete, inaccurate or misleading income information may be removed from consideration.

Income eligibility is only one part of the application process. Final recipients will be selected based on financial need, medical eligibility, completed documentation and available program resources.
