In addition, there is a $300 fee added to the total cost if there is any history of prior abdominal-pelvic surgeries, such as C-section, tummy tuck, etc.

The payment for the balance of the surgery fee is due by the pre-operative appointment. If you choose to pay by check, the payment must be received at least two weeks before the surgery date. The $1500 tubal ligation reversal surgery deposit is non-refundable. If the surgery needs to be rescheduled within two weeks of the surgery date, then the $1500 deposit is forfeited and another Non-Refundable $1500 Deposit is required in order to get a new date.

There is a $300 consultation fee if wish to visit the office for a face-to-face consultation. However, you can also avail of telephone and Skype consultations, which are available at a discounted rate of $150 for patients that do not live in North Florida.
